This Privacy Policy explains what information we collect on this website, why we collect it, and the choices you have. It applies to the Sunset Hills Imaging website and the online services it connects you to (booking, order submission, and the contact form).
We only collect the information needed to run the site and respond to you. We do not sell your information, and we do not share it with outside parties beyond the service providers described below.
Information You Provide
- Booking requests. When you request an appointment online, your name, contact details, and the scan you’re booking for are processed by our booking provider, IntakeQ. See “Third-Party Services” below.
- Contact form. The “Send Us a Message” form on our contact page sends your message, along with your name and email address, through IntakeQ to our clinic’s secure system.
- MRI order form. The printable order form on our order form page captures the details a provider needs to submit an order, plus the visitor’s IP address. See “Order-Form IP Capture” below.
Information We Collect Automatically
- Google Analytics 4. We use Google Analytics 4, a web analytics service provided by Google LLC, to understand how visitors use our website — which pages are visited, roughly where traffic comes from, and how the site is navigated. Google Analytics uses cookies to collect this information and may store it on Google’s servers. You can opt out of Google Analytics by installing Google’s Analytics opt-out browser add-on or by blocking analytics cookies in your browser settings. Google’s handling of this data is described in Google’s Privacy Policy.
- Standard web logs. Like most websites, our hosting provider records basic technical data — IP address, browser type, pages requested, and timestamps — to keep the site running and secure. This data is not used to identify you.
Third-Party Services
- IntakeQ. Our online booking widget and our contact form are provided by IntakeQ, a third-party scheduling and forms service. When you book an appointment or send us a message, the information you submit (name, contact details, appointment details, and the contents of your message) is transmitted directly to IntakeQ’s systems and shared with us through their dashboard. This is how we receive your booking or message — we do not see it before IntakeQ does. IntakeQ’s handling of your data is described in IntakeQ’s Privacy Policy. We do not use IntakeQ to store data beyond what is needed to manage your booking or message.
- Google Analytics. See “Google Analytics 4” above.
- Google Maps. Our contact page shows a static map placeholder. The interactive Google Maps embed loads only when you click “Load interactive map,” at which point Google may collect location-related data under Google’s Privacy Policy.
Order-Form IP Capture
When you open the MRI order form page, a small script records the public IP address your device is connecting from. That IP address is printed on the generated order document so that submitted orders can be traced back to a source if needed. The IP address is not stored on our server — it appears only on the order document your provider prints and submits. The same script falls back to the third-party service api.ipify.org if our own endpoint is unavailable; in that case the IP address is shared with api.ipify.org solely for the purpose of returning it to the page.
How We Use Your Information
We use the information we collect to:
- Receive and respond to booking requests and messages you send us.
- Receive and process MRI order forms submitted by providers.
- Operate, secure, and improve the website.
- Understand how the website is used (analytics).
We do not use your information for advertising, and we do not sell it.
Data Retention
We retain the information you submit only for as long as needed for the purpose it was collected — for example, to complete and follow up on a booking, respond to a message, or maintain records we are legally required to keep. Standard web logs are retained on a short rotation.
Children’s Privacy
This website is intended for adults arranging their own medical care. We do not knowingly collect personal information from children under 13. If you believe a child has provided us with personal information, contact us and we will delete it.
Links to Other Websites
Our website links to third-party sites (for example, IntakeQ, Google Maps, and our Google reviews listing). Those sites have their own privacy policies, and we are not responsible for their practices.
Security
We use industry-standard measures to protect the information transmitted through this website, including HTTPS encryption for everything sent over the network. No method of transmission over the internet is 100% secure, but we take reasonable steps to safeguard the data you send us.
Your Choices
- Analytics. You can block or opt out of Google Analytics as described above.
- Bookings and messages. Information you submit is collected at your request. If you’d like us to delete a message or booking record we hold, contact us using the details below.

We may update this policy from time to time. When we do, we will change the “Last updated” date at the top of this page. You are advised to review this page periodically.
