St. Louis MRI Cost Without Insurance

St. Louis MRI Cost Without Insurance

If you don’t have insurance, or you have a high-deductible plan, the first question is almost always the same: what does an MRI cost out of pocket? The honest answer in St. Louis is that it depends entirely on where you go. A hospital can quote you a number over $1,500. We charge a flat $399 for every scan we run. This post explains why the gap is so wide and what the price does and doesn’t include.

What Hospitals Typically Charge

When you pay cash at a hospital-based imaging center, you’re usually paying the facility’s list price, not the negotiated rate an insurer would pay. For a single region like a lumbar spine or knee MRI, that list price routinely lands between $1,500 and $3,000 in the St. Louis area. That number often comes with surprises on top: a facility fee, a separate radiologist reading fee, and sometimes a third bill from a billing service.

None of those are services you didn’t ask for. They’re just how hospital billing is structured. The imaging center charges for the room and the machine, the radiologist group charges for the read, and you get multiple statements for one scan.

The Deductible Trap

Here’s the part that surprises people with insurance: if you haven’t met your deductible, using insurance may not save you anything. Your plan’s negotiated rate still applies, and you pay it out of pocket until the deductible is satisfied. That negotiated rate for an MRI is often more than $399.

That’s why we hear from patients who have insurance and still choose to pay cash. It’s not that insurance is bad. It’s that for one MRI, the flat self-pay rate can be less than what you’d owe toward a deductible anyway, and you get a single, predictable price instead of a bill that arrives in pieces.

What Our $399 Includes

We’re an independent clinic in St. Louis, and our pricing is simple: every non-contrast scan is a flat $399. That covers the scan itself on our Siemens 1.5T, the board-certified radiologist’s read, and a CD of your images. There is no facility fee, no separate reading fee, and no surprise bill. You pay once, and that’s the price.

A few honest notes. We don’t bill insurance at all, so the $399 is the full picture. We perform non-contrast studies only, which means no IV and no contrast dye. And we accept HSA and FSA cards, so if you’re paying with a flexible spending account, it works like cash for us.

What Most Patients Actually Scan

The most common study we run is a lumbar spine MRI — the low back. It’s a good example of the price difference. At a hospital, that study without insurance can easily run four figures. With us, it’s $399, you go in feet first, your head stays outside the scanner, and the scan takes about 25 minutes.

If you need more than one region scanned, each study is its own $399, and we’ll be straight with you about which ones make sense for your symptoms. More scans isn’t automatically more clarity. Sometimes the right answer is one region, sometimes it’s two or three, and the honest recommendation depends on what your doctor’s order says and where the symptoms point.
